How To Access Patient Care at Park Leys Medical Practice

We have three primary ways for you to access appointments at any of our surgeries.

1. Non Urgent

You can now request a doctor’s appointment, fit (sick) note, ask about test results, or submit an admin query through our new online system. This is called e-consult.

This link below will allow you to send the practice a medical or admin query which will be responded to within 3 working days. It will save you sitting on the phone and speaking to a receptionist.

2. Urgent

If you need to make an urgent appointment or prefer speaking to someone over the phone, please call 024 7633 2636 between 8:30 and 18:00, Monday to Friday, excluding bank holidays.

The practice offers a wide choice of telephone and face to face appointments with a selection of GPs, PAs, ANPs, Nurses and more. For a full list of clinicians, you can click here. Our receptionists are trained to book our patients in with the most appropriate clinician. As such, they will ask some basic questions about the nature of your problem to ensure you receive the best possible care. We offer a choice of same day and advance bookings; urgent issues will always be prioritised to be dealt with the same day. However, if your problem can wait you may be offered an appointment for another day when a more suitable clinician is available.

3. NHS App

You can now access our services via the NHS app. You can use the NHS App wherever you are, at any time.

The NHS App will not replace existing services and you can still contact us in the usual ways if you prefer.

To gain full functionality you will need to apply for access to online services. You can do this by coming into the surgery and filling out your contact information including the email you intend to use with the app.

Blood Tests & Vaccinations

A phlebotomy (blood taking) clinic is held every day at our main surgery and throughout the week at our other surgeries. We provide many vaccinations to our patients including a comprehensive travel vaccination service. When requesting this service you will be asked by the receptionist to complete an electronic pre-travel questionnaire to assist the nurse.

Times of surgeries and clinics for these services vary so please contact the surgery for the latest information.

Home Visits

Doctors will make home visits in an emergency to housebound patients and those unable to visit any of the surgeries.

If you require a home visit, please telephone before 10:30.

It is likely a Doctor will need to call you before making a decision on whether a home visit is appropriate, please ensure you provide us with a contact number we can reach you on. 

Extended Hours Service

We are happy to offer patients appointments to be seen in the evening or weekend where applicable.

We offer as small selection of extended hours appointments Monday to Friday 18:30-20:00 and Saturdays 9:00-17:00. When requesting an extended hours appointment, you may be asked to attend one of five other surgeries in our Primary Care Network. Click here for more information.

Test Results

The wait time of test results can vary depending on the nature of the test and current demand of the service. For blood test results we ask patients to wait 5 working days before contacting the surgery. If you have been to the hospital for a test you may have been advised on how long it takes for the GP to receive your results. You are welcome to call at any time while the surgery is open or use our e-consultation service to ask for test results.

For urgent test results a clinician will contact you, however for non-urgent and normal test results it is the responsibility of the patient to contact the surgery. 

Forms and Questionnaires

If you have been asked to complete a form or questionnaire by a member of  staff, please return it to any of our surgeries or email a scanned copy to our practice email. Please let the receptionist know of any urgency involving your forms, as it can take over 5 working days to be viewed by a member of our admin team during busy periods.
